FOCUS Connect (FOCUS) is a highly respected Not-for-Profit organisation operating within the Aged Care and Community Services sector in Sydney’s Macarthur Region. With a workforce of approximately 130 employees, 230 CHSP and 73 HCP participants, FOCUS Connect had identified the urgent need to update their IT systems to enable close alignment with the significant changes coming in the aged care reforms. FOCUS Connect enlisted a specialist vendor SynerGisteX to assist with the introduction of a new Care Management System called “VisualCare,” aimed at enhancing operational efficiency, preparing the business to embrace change, sector reform and improving client experiences.
In the aged care and disability service sectors, providers commonly lack dedicated full-time IT staff, requiring them to engage external specialists such as SynerGisteX to deliver initiatives. This strategy reduces costs, lowers risk, removes the need for expensive under utilised IT staff and speeds up the implementation of affordable IT solutions across the organisation. To deliver the project FOCUS Connect used SynerGisteX to provide interim project management and IT consulting and act as the liaison for FOCUS Connect with VisualCare. This arrangement facilitated a smooth transition from a disjointed collection of five legacy software platforms to a single, integrated system centred on VisualCare. SynerGisteX lead interactions with VisualCare’s implementation, the finance department, and backend teams throughout the process, reducing complexity for FOCUS Connect and ensuring a seamless transition with minimal operational impacts.

– Review of legacy databases to determine data extracts required
– Comprehensive project management throughout the rollout
– Backend IT data migration and configuration via system administration
– Acting as FOCUS Connect’s advocate in communications with VisualCare teams

– Data migration services covering data export/import, cleansing, formatting, and mapping

Through the deployment of VisualCare, in collaboration with SynerGisteX, FOCUS Connect laid the groundwork to swiftly adapt to ongoing Aged Care reforms, delivering an agile capability. Due to these concerted efforts, the integration of VisualCare into everyday operations was conducted smoothly and effectively. The project was completed successfully in a short timeframe, and FOCUS Connect has seen substantial benefits leading to continued engagement with SynerGisteX for further ongoing investment in VisualCare related modules and features to deliver continuous improvement. SynerGisteX has been recognised by FOCUS Connect as experts in the area of complex implementations , continued IT strategy and integrations and was invited to represent FOCUS Connect at the recent VisualCare Conference in Queensland, which proved to be highly beneficial for all parties involved.
